In accordance with the provisions of Sections 45, 46 and 57 of the Motor
Vehicles Act, 1939, I/We, the undersigned, hereby apply for a permit under
Section 42 of that Act in respect of a service of stage carriages as hereinunder

the accommodation for passengers in any vehicle will be replaced by
