Application for a permit in respect of a particular contract carriage to be regularly so used.

            In accordance with the provisions of Section 45, 49 and 57 of the Motor Vehicles Act, 1933, I/ We the undersigned hereby apply for a  permit under Section 42 of that Act in respect of a contract carriage as hereunder set out.
